Arkansas Student Disclosure Form

The State of Arkansas requires Clinical Pastoral Education International (CPEI) prior to enrolling students from Arkansas to meet specified requirements outlined in the Exemption from Certification Form 17B granted to CPEI.

A copy of the student disclosure statement must be signed and dated by students that inform them prior to enrollment of the following: Clinical Pastoral Education International offers faith-based related training courses and certifications that are not transferable as academic degree programs to colleges and universities accredited by agencies recognized by the United States Department of Education. This signed & dated student disclosure statement acknowledges that the State of Arkansas has granted CPEI an Exception from State Certification and shall be maintained by CPEI as part of the student’s record.
